The Wisconsin Department of Health Services (DHS), Division of Quality Assurance (DQA), recently provided advance notice to nursing home providers about the upcoming annual reporting period.
Under Wisconsin law, nursing home licensees are required to submit an annual report using the DQA form and pay the required fee. On or before September 1, 2026, all Wisconsin nursing homes will receive their annual report packets electronically by email.
Review and Return Annual Report Forms by November 1
Upon receiving the packet, providers should carefully review the information included in the annual report and document any necessary changes using the forms provided. Completed forms must be returned to DQA no later than November 1, 2026, at dhsdqabnhrclicensing@dhs.wisconsin.gov.
Failure to submit the required information may result in revocation of a facility's license.
Annual Facility Fee Also Due November 1
In addition to completing the annual report, nursing home providers must pay the annual inpatient health care facility fee. An invoice showing the amount due will be included in each facility's annual report packet.
Payment is also due November 1, 2026. Payments received after that date will be assessed a $10-per-day late fee.
