Wisconsin DHS Update: Assisted Living Survey Readiness & Requirements

Wisconsin DHS has released updated guidance on survey readiness, surveyor access to records, and provider expectations during assisted living surveys for Adult Family Homes (AFHs), Community-Based Residential Facilities (CBRFs), and Residential Care Apartment Complexes (RCACs).

Providers should ensure records are current and organized, staff are prepared for surveys, and surveyors receive timely access to facilities and requested paper or electronic records. Records requested at the entrance conference must be provided within two hours of its conclusion. Providers are also expected to maintain professional communication, cooperate with surveyors, and have clear processes for addressing concerns.

Facilities should review the guidance and establish procedures to support ongoing survey readiness and compliance.

Wisconsin DHS DQA Provider Portal Update: The Wisconsin Department of Health Services has a Provider Portal Help Request Form available for facilities experiencing technical issues with the DQA Provider Portal or MyWisconsin ID. The form is intended for technical support requests, not regulatory or general process questions. Providers already using the portal should submit questions through the Portal Communication Center. DQA representatives aim to respond to help requests within two business days.
