Register your facility for the 2025 MDS Innovation Team membership. As a reminder, only LeadingAge Wisconsin provider members/subscribers are allowed to participate in these meetings. This registration is by facility, not at overall organizational level. Please noteIt is possible that multiple individuals from your organization may receive this notice. Please coordinate your MDS team application so that each facility completes only one application - thanks! 

 

The $125 annual facility fee helps offset room rental fees, lunch meal, and handouts.

 

Spotlight on Amy Veldt

 

Amy Veldt, RN, RAC-CT, has 40 years of experience in long-term care, 27 years with the RAI process. She started her own health care consulting business in 2005, offering audits, training, and education for nursing homes and assisted living facilities. The goal of Pinnacle Innovative Healthcare Solutions is to assist the facility in achieving and maintaining regulatory compliance, effective reimbursement systems, and resident-centered care delivery. Amy also will provide innovative guidance to ensure efficiency of systems, teams, and individuals.

 

Amy continues to offer training and educational sessions statewide, as well as on-site consultation, education, and guidance. She often is a featured presenter at the LeadingAge Wisconsin educational events. Amy started the MDS Innovation Team in 2006 and, with the support of LeadingAge Wisconsin, continues to facilitate the meetings in locations throughout the state of Wisconsin.
