Please ensure that your facility has paid membership to the MDS Innovation Team for 2026 prior to attending the next meeting. If you are unsure if your facility has paid for 2026, please email Taylor Raether.

RSVPing by the deadline is required. We will not have extra meeting handouts, seating, or lunch available for attendees who did not register in advance.

DIRECTIONS: The meetings will be held at Capernaum Community Center, which is located in the middle of Grace Lutheran's River Pines facility and Prairie Pointe facility (one building with two facilities). Each facility has an entrance to the building and you must enter Capernaum Community Center from one of the facility entrances. River Pines is located at 206 N. Willson Drive, Altoona and Prairie Pointe is located at 286 N. Willson Drive, Altoona. Parking is available on all sides of the building.
